Helpers can proceed with eligible writes while pending rule checks and “always” approval settings continue to block them. No actionable merge-blocking risk remains after normal checks.
Security architecture risk:
Moderate · up to 446c3

Background work can change stored files or other internal data while a newer restriction is still being checked. The exception is limited to internal writes, and saved restrictions and existing approval checks still constrain it.
Retained concerns
- Medium · security · inferred: An older pending Ask proposal can let an unattended internal write proceed while a newer check for the same tool is still running. The exception does not distinguish this overlapping state from a settled pending Ask. A later Never proposal can restrict subsequent calls but cannot retract the completed write, weakening the previous fail-closed rule-check boundary.
Security review details
-
Security Blast Radius
- inferred — The affected authorization decision is session-scoped, but its effects can reach application-owned state exposed by selected internal-write tools, rather than only the regression test's file. The exception does not directly admit send or execution risk. No expansion to another tenant, environment or external identity was established by the inspected paths.
Security Findings and Attack Paths
- inferred — A running helper processes its supplied goal and context under the originating session. If that session already has a pending Ask proposal and a newer relevant check is evaluating a restriction, model-directed internal writes can pass the new exception before the result arrives. This requires the overlapping state and no independent blocking control; an active check alone, without a stored Ask proposal, is insufficient. Public unauthenticated reachability was not established.
- observed — The retained mode-always condition is not established as a newly exposed bypass in the inspected production callers. Ordinary subagents check approval requirements before reaching the exception. Swarm workers use a null session, so pending chat proposals cannot activate it. The base already allowed otherwise unrestricted no-UI calls without consulting the attended approval decision path.
Trust Boundaries and Controls
- observed — Subagent policy executes before the pending-proposal exception and rejects send-or-higher risk and calls requiring approval. The planner also retains read-only restrictions, lasting rules and outside-content controls for outgoing effects. These controls narrow the exception but do not synchronize eligible internal writes with an in-flight rule check.
Resilience and Maintainability Implications
- observed — Check markers are removed in finally cleanup, and proposal decisions are serialized. Neither mechanism spans tool admission and execution. Runnable tools may start concurrently; cancellation stops unfinished tasks but supplies no shared compensation for completed writes. A later restriction therefore cannot restore the prior state through these mechanisms.
Hardening Proposals
- proposed — Distinguish a settled pending Ask from an active relevant check, and keep the latter fail-closed. Bind admission to a rule-state generation and revalidate before effects begin. Add a regression covering an existing pending Ask followed by a concurrent Never check, including cancellation and multiple planned writes.

Full details: Title check
-
Explanation
The title "fix(agent): a pending rule guess no longer stops a helper's file_write" matches the main change. The raw summary and PR objectives show the change lets unattended helper calls proceed when the only blocker is a pending Ask chat-rule suggestion. The title names the agent area, the pending rule guess, and the affected tool, so a teammate scanning history can identify the primary change. It is a conventional-commit style title of 70 characters and 11 words, which is within the expected concise range.

Explanation
Issue
#491requires unattended helpers on a fresh home to write the internal file or ask without failing. The current code allows an internalRisk.writewhen the only blocker is a pendingASKsuggestion and approvals mode is notalways. It still stops for user-set rules, home-opening calls, ask-first routines, pendingNEVERsuggestions, and matching rule checks in progress. Tests cover the allowed write and each listed stop condition. The reported qwen3:8b run is manual validation, not a coding requirement.

Explanation
The changes stay within Issue
#491. They add pending-rule classification, unattended helper handling, safety guards for active checks andalwaysmode, regression tests, user-facing refusal text, and changelog entries. These changes directly support the issue or preserve its required safety behavior. No unrelated change is demonstrated.

Thanks @azmi2409! Your root-cause analysis is spot on: the "Ask" comes from a pending rule card, not from a default rule. I tested on a real engine (Claude Sonnet chat, qwen3:8b and Sonnet helpers).
On a truly fresh home, helpers can already write files on main. The failure only shows when the user's own words create a card. For example, "Do not write any files yourself" makes a pending Never card for file_write. With this PR, helpers write files while that Never card is still waiting for an answer. In the chat, the same card would make Sentient ask first, so helpers end up less careful than the chat itself. "Never save files to my computer, and have helpers research X" would then save files silently.
The good news: explicit Ask and Never rules, and exec and sends, still behave correctly.
Here's the direction we'd like:
- Pending Ask card + an internal write (writing inside Sentient's own files folder): helpers may go ahead. That's the case #491 is about.
- Pending Never card: keep stopping helpers, exactly as main does.
- Fix the message for the stopped case. It currently says "set to Ask… Change it in Settings" (
tools/rules.py~151 vialoop.py~1266), but nothing in Settings is set to Ask; a card in the chat is waiting for an answer. Something like "Waiting for your answer on the suggested rule in this chat" would be accurate, and the main assistant can then do the write itself and ask once.
Tests for both cards (Ask: helper writes; Never: helper stops with the new message) would lock it in. Happy to talk it through!
…message A pending Ask card now only lets a helper's internal write go ahead (existence-master#491); a pending Never card stops helpers exactly as main does, and the stopped case says a suggested-rule card is waiting for the user's answer in the chat instead of claiming something is set to Ask in Settings.

Thanks @azmi2409, this is exactly the behaviour we wanted. On a real engine (Sonnet chat; helpers on Sonnet and qwen3:8b):
- a pending Ask card let two helpers write their files;
- a pending Never card stopped them, with the new message;
- explicit Ask and Never rules still won.
One optional polish for later: the message speaks to the user ("your answer") and then about them ("ask the user"), and the "Settings" mention led the assistant to say the card was in Settings. Something like "The user hasn't answered the suggested rule for X in this chat yet, so this wasn't done. Do it in the main chat and ask them once." would read cleaner. Approving and merging!

@azmi2409, one more small round, sorry. I approved before noticing CodeRabbit's two open threads, and both are right:
Both come with CodeRabbit's suggested code. A test for each, then reply to and resolve the threads, and it's ready to merge. Everything else in my approval stands! |
A rule check still running for a tool must stop a helper's internal write, even when an older Ask proposal for the same tool would let it through: that check may still become Never. Add ChatRules.is_checking() and check it in _pending_guess() before the stored-proposal exception. Also keep the pending-Ask exception out of approvals mode always, where the user asked to approve every write. Tests: a running-check test and an always-mode test in tests/test_approval_rules.py.

Thanks @azmi2409, both fixes hold up on a real engine. Re-tested 8c6db9b end to end: fresh home, chat on Claude Sonnet (Claude Code), helpers through delegate_tasks on Sonnet too. The rule cards came from real chat messages, and every helper file_write targeted Sentient's own files folder.
(i) Pending Ask card: the helpers' writes go through. "Always ask me first before you write any files." made a pending Ask card for file_write. I left it unanswered, then asked for two helpers. Both file_write calls succeeded ({"saved": "notes-a.txt" ...}, {"saved": "notes-b.txt" ...}), both files are on disk, and the card is still pending.
(ii) Pending Never card: the helper is stopped. "Never write any files about my medical history." made a pending Never card for file_write. The helper's file_write failed with the new message: Waiting for your answer on the suggested rule for "File write" in Files in this chat. It was not done, and nothing in Settings is set to Ask. .... The main chat then asked before its own file_write. I denied it, and the reply said plainly that nothing was written.
(iii) Never check still running, plus an older Ask proposal: the helper is stopped. To hit the window I made the check slow. In a new chat, "Always ask me first before you write any files." first left a pending Ask card. Then I pointed the fast role at a small local OpenAI-compatible server (lm_studio/slow). That server waits 120 s and then answers {"keys":["file_write"],"rule":"never"}. Then I sent "From now on, never write files into my Documents folder. Separately, ... have one helper write notes-d.txt". The reply waited its 20 s and moved on while the check kept running:
- 23:27:51 the check reached the slow model
- 23:28:43 the helper's
file_writewas stopped with the pending-card message, and no file was written - 23:29:51 the Never card landed (
f70e917a,never,file_write), next to the older pending Ask card91ad7a07
Same steps on the previous commit 446c39a: the write went through. At 23:35:38 the helper saved notes-d.txt while the Never check was still running, and the Never card landed at 23:36:42. So this commit closes the hole CodeRabbit found.
(iv) Approvals mode "always" plus a pending Ask card: no exception for the helper. The chat asked before delegate_tasks (I allowed it). The helper's file_write was stopped ('file_write' needs the user's approval, which a subagent cannot ask for...), and no file was written. As you said, the subagent policy catches this before _pending_guess. Swarm workers and proactive runs have no chat session, so they never reach the pending-card path either. Your policy-less unit test covers the mode != "always" guard itself, and it is the right defence for the future.
Other checks
tests/test_approval_rules.py,test_rules_from_chat.py,test_subagents.py,test_rule_levels.py: 110 passed locally.- CI is green and the CLA is signed.
- Both CodeRabbit security threads are answered and resolved, and I agree with both resolutions. There are no unresolved threads.
- It merges cleanly with current main.
Approving.
Fixes #491.
Root cause. On a fresh home no default rule asks for
file_write(it is an internal write; the chat itself runs it without asking in mode "ask"). The "Ask" came from a pending rule suggestion in the chat: while a proposal is undecided, or its check is still running,chat_rulereturns"ask"and_plan_callturns that intorule = "ask"inside the helper's loop. Helpers run unattended (use_approvals=False, cannot pause), so the call failed with "set to Ask, and this work can't stop to ask", even though the approvals engine and the subagent policy both cleared it.Fix. In the unattended branch of
_plan_call(sentient/agent/loop.py), when the "ask" comes only from a pending chat guess — no lasting ask/if_asked/hand-off rule, not a home-opening call (ADR 0028), not a routine set to ask first (#161) — the call goes ahead instead of failing. A guess only ever adds a question, and this run has nobody to ask. A rule the user actually set still stops helpers; that path is unchanged.Tests.
test_pending_rule_guess_does_not_stop_a_helper_file_write(tests/test_approval_rules.py): creates a real pending Ask proposal forfile_writeviachat_rules.check, then runs a helper loop the waySubagentManager._rundoes (no approvals UI, subagent policy, can't pause); asserts the file is written with no error. Changelog entrychanges/491.fixed.mdadded.Real test. Integration level only (scripted
FakeProvider, no network/model). I could not do the issue's "real run on qwen3:8b" in this environment; the "have two helpers each write a short file" scenario on a fresh home is worth a maintainer run.Summary by CodeRabbit
