What and why

Fixes #492.

#407 corrects replies that claim a browser click that never happened or was declined. The same false claim happens with the other action tools: "I sent the email", "I saved the file", "I booked it", "I turned on the lights", when the call was declined, failed, or never made.

This extends #407's approach (sentient/agent/outcomes.py) to those tools, reusing the outcome checks from #165/#380:

  • every call of an action tool with a clear outcome (send email/message, file write, calendar create, Home Assistant calls) is now recorded by code in result.action_calls, mirroring browser_clicks; declined calls are recorded too, since the outcome check skips them by design
  • action_call_history() rebuilds the same evidence from earlier chat turns, mirroring browser_click_history(), so a reply summarizing a previous turn's successful action is never "corrected"
  • new unmade_action_note() corrects a reply that claims an action the run's own records contradict, with the same conservative rule: never correct a true reply, and when unsure, leave it alone (shared verbs like "sent" stay ambiguous without a naming noun; ordinary chat verbs like "wrote a summary" or "added context" need a naming noun; other people's actions, negations and hypotheticals are left alone)
  • the correction is appended at the same two final-text sites as the click correction, so voice replies and channel replies are covered too (both speak/post that same text)

How I tested it

  • .venv/bin/python -m pytest tests/test_agent_loop.py tests/tasks/test_outcome_checks.py -q — 34 passed (7 new: a never-sent and a declined-send integration test through the full agent loop with FakeProvider, plus unit tests for true-reply protection, declined/failed/never-made wording, the conservative leave-alone cases, and history)
  • ruff check sentient/agent/outcomes.py sentient/agent/loop.py tests/test_agent_loop.py — clean; git diff --check — clean
  • full suite on this branch vs base 52c7e74: identical failure sets (all pre-existing environmental issues in this sandbox, e.g. an httpx Invalid port error in the channels tests) — no regressions

Real test

  • Setup: pytest with FakeProvider driving the real agent loop (tool planning, approval deny, final text and persisted-message assertions); no model involved.
  • Model: none — no Ollama/GPU in this sandbox, so the issue's qwen3:8b end-to-end run was not possible here; the FakeProvider integration tests cover the same loop paths as fix(browser): correct unmade click claims #407's pytest evidence.

Checklist

  • Tests added or updated, and pytest passes locally for what I touched
  • ruff check sentient tests is clean
  • docs/API.md updated if the desktop <-> engine contract changed — not needed (LoopResult is internal; no event or route changes)
  • A changelog entry in changes/ (changes/492.fixed.md)
  • No keys, tokens or personal data in the code, tests or screenshots

Summary by CodeRabbit

  • Bug Fixes
    • Improved accuracy when reporting email, messaging, file, calendar, and smart-home actions.
    • Corrected replies that claim an action succeeded when it was declined, failed, or not attempted.
    • Clarified when an action’s outcome cannot be confirmed.
    • Preserved accurate reports of completed actions, including actions from earlier conversation turns.
    • Improved handling of ambiguous statements and unsupported action claims in voice and channel replies.

Walkthrough

The chat loop records outcomes for supported action calls and carries prior outcomes into later turns. It checks replies for unsupported action-completion claims and adds correction text when needed. The change also adds a script that creates and verifies a GitHub branch.

Changes: Action outcome corrections

Identify claims and action evidence
sentient/agent/outcomes.py
Action records include outcome and target evidence. Claim analysis distinguishes action types, named targets, self-attribution, negation, hypothetical claims, and other-person claims.
Track outcomes and correct replies
sentient/agent/loop.py, sentient/tasks/executor.py
The loop loads prior action history, records current outcomes, and checks final and step-limit replies. The executor records approved-call evidence and reconstructs history when resuming.
Validate action corrections
tests/test_agent_loop.py, changes/492.fixed.md
Tests cover action claims, outcomes, targets, stored results, incomplete history, and resumed tasks. The release note describes corrections for voice and channel replies.

The false-claim correction can occasionally stay silent for a handed-off or try-run action, or wrongly correct a true reply after a very long truncated result. These are narrow edge cases with safe fallbacks. The one-off branch-push script is not part of the shipped product. Merging is acceptable with these items as follow-up.

Explanation

Issue #492 is open and directly linked. sentient/agent/outcomes.py records clear action outcomes and applies conservative claim matching for email/message sends, file writes, calendar events, and Home Assistant actions. sentient/agent/loop.py records current calls, rebuilds prior-turn history, and applies correction at final reply paths. Tests cover declined, failed, never-made, successful, uncertain, history, voice/channel loop paths, and true-reply protection. The reported qwen3:8b run is an environment-dependent completion check, not a coding requirement that this review can require instead of automated coverage.

Full details: Out of Scope Changes check

Explanation

push-492.py is unrelated to the runtime correction in issue #492. It reads local HEAD, calls the GitHub API, creates or force-updates branch fix-492-false-claims in azmi2409/Sentient, and verifies the remote tree. It does not implement, test, or document action-claim correction.

Resolution

Remove push-492.py from this pull request. Keep branch-publishing automation outside the issue #492 implementation unless a separate linked coding requirement covers it.

138-145: Verify that passive claims cannot contradict a successful call of a different kind.

_owns_passive_claim returns True when only one kind's verb matches. For "The file was created.", only file matches. For "The event was created.", file and event both match, and the noun check then picks event. For "The note was created.", _KIND_NOUNS["file"] matches "note", so the claim is attributed to file. If the user created a calendar note with an event tool, the reply is not corrected, which is safe. If the note was saved by another tool that has no recorded file call, the code does not reach this path, because the any(call.get("kind") == kind ...) guard requires a prior file call. Static reading shows no defect.

578-609: 📐 Maintainability & Code Quality | 🔵 Trivial | 💤 Low value

⚠️ Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

A targeted claim can pass as true when the only success was a different target.

When a named claim matches no call and a successful call of the same kind exists, _action_note returns "". Example: "I sent the email to Bob" is not corrected when only an email to Alice succeeded. This follows the "when unsure, leave it alone" rule from issue #492, so it is an accepted trade-off. It means a wrong-recipient claim is never corrected. Document this limit in the docstring so maintainers do not read it as a bug.

sentient/agent/loop.py: 2

346-382: Historical calls lose the real target of declined actions and can mis-record ok status.

action_call_history derives ok only from the stored result. It does not use the approval row's target or the check result. A successful email with a result such as {"sent": [...]} has no error, so ok is True. That is acceptable. A tool whose result text is truncated or not JSON becomes res = {}, so ok is True and declined is False. The comment says this errs toward never correcting a true reply. The same fallback also marks a truncated failure as successful. A later false "I sent it" claim is then not corrected. This is a safe-side trade-off, so no change is required.

1025-1032: The correction is applied after the browser-click correction, so two corrections can stack.

When a reply makes both a false click claim and a false send claim, both notes are appended. This is acceptable. The duplicate guard uses f"Correction: {note}" not in text_acc, which works for a copied correction. LGTM.

Welcome back @azmi2409, and thanks for taking on #492 (and #491 in #496)! I've assigned both issues to you so nobody else picks them up. Testing both on a real engine now; reviews to follow.

Thanks @azmi2409, this is a careful extension. True sends and saves held up perfectly in our real runs, and declined, failed and never-made ones were corrected, including in voice and on Telegram. 🙌

On qwen3:8b, though, true replies about actions this PR doesn't track got corrected (5 times in 44 turns):

  • "I saved that your balcony faces west." right after a real memory_remember became "Correction: I didn't write the file, so nothing changed."
  • "I've scheduled a task…" after a real task became "I didn't create the event".
  • "I set up a daily task…" and "I ran a quick check and nothing was sent." became "I didn't run the home action".

The cause is verbs like set/ran/done/scheduled/saved counting as proof of an email, file, event or home action (outcomes.py ~45-51 _CLAIM_VERBS, ~121 _owns_claim), plus "I didn't {do it}" whenever no recorded call matches, even when records exist (~598/600). Could you:

  1. Name the action: only correct a never-made claim when the sentence names the action (an email or message noun, a file name, a calendar word, a device), or when this turn actually tried that action.
  2. Treat unknowns as unsure: a resumed task (tasks/executor.py _run_approved_call, where task loops get no history) and calls outside the history window should count as "unsure", not "never made". A FakeProvider repro of a resumed approved send ends with "I sent the email to Arjun… Correction: I didn't send the email".
  3. Match the full target: keep the full address or file name when matching (~544 cuts at ".", so "arjun@example.com" and "plan.md" never match), and say "couldn't confirm" for uncertain calls rather than "didn't send".
  4. Accept ’ (the curly apostrophe) in the negation checks: "The email wasn’t sent." after a decline gets corrected. That gap is on main too (#407); we've filed it separately, so fixing it in your new checks is enough here.
  5. Soften the changelog line: "True reports are never corrected" isn't quite true yet.

Tests for the memory, task, reminder and resumed-task cases would lock it in. #494 is landing first; it only touches the click path, and your PR already includes its two fixes, so after #494 you just need to merge main. Happy to help!

…ction

- Only correct a never-made claim when the sentence names the action
(email/message noun, file name, calendar word, device), or when this
turn actually tried that action: bare verbs like saved/scheduled/set/ran
also describe untracked actions (memory, tasks, reminders).
- Resumed tasks: the approved call's record becomes the resumed segment's
history, so a true summary is never corrected; calls outside the loaded
history window count as unsure, not never made.
- Keep the full address/file name when matching targets (no cut at '.');
uncertain calls say 'couldn't confirm' rather than 'didn't send'.
- Accept the curly apostrophe in the new negation checks.
- Soften the changelog line; add tests for the review's cases.

sentient/agent/loop.py:368-381
🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Parse the leading JSON of a stored tool result before you derive ok.

_record sometimes appends text after the JSON before it persists a tool row:

  • If a read-back check fails, outcomes.model_note(check) adds "\n<note>" (Line 1638-1643).
  • _tool_content can cut a long result.

In both cases json.loads raises, res becomes {}, and ok becomes True. A call whose read-back says "the light is still off" is then stored as a success in action_call_history.

This causes two problems in a later turn:

  • A false "I turned on the lights" claim gets no correction.
  • has_success in _action_note hides corrections for claims about other targets of the same kind.

Fix: decode only the leading JSON value, and treat an appended outcome note as a failure.

Comment thread sentient/tasks/executor.py Outdated
…leading JSON of stored results

- tasks/executor.py: on resume, reconstruct result.action_call_history from the whole resumed checkpoint (via action_call_history) instead of appending only the approved call. _run_approved_call already records the approved call's result in the checkpoint, so reconstruction includes it plus every earlier action call; a true summary of any of them is never 'corrected' (existence-master#492 review).
- agent/loop.py: action_call_history decodes the leading JSON of a stored tool result instead of requiring the whole content to parse. Text appended after the JSON is an outcome note (existence-master#386, read-back said it did not work) and now counts as a failure; a '[Result cut/shortened ...]' marker is not an outcome and stays a success, so a true reply is never corrected for it.
- tests: cover the outcome-note/cut-marker parsing and the whole-checkpoint rebuild for a resumed approved send.

Do not treat truncated JSON as a failed action.

_tool_content persists the cut result. If the cut occurs inside the JSON, raw_decode fails and action_call_history treats the whole body as an outcome note because it starts with {. This sets ok=False, so a later true claim can receive a false correction.

The correction must also preserve an explicit "ok": false found before the cut. result_problem({}) returns None, so ignoring the cut marker without extracting that field would incorrectly mark a known failure as successful.

🐛 Suggested fix
         raw = str(row.get("content") or "")

Update the JSON parsing and outcome-note handling in action_call_history so a
result cut inside its JSON is not treated as a failed action. When decoding
fails on a cut result, preserve any explicit “ok” value found before the cut,
especially false, and exclude the cut marker from noted_failure; add regression
coverage for cut successful JSON and cut JSON with “ok”: false.

Thanks @azmi2409, big step forward! On qwen3:8b (60 turns) and Sonnet (38):

  • all five earlier wrong corrections are gone;
  • true sends and saves stand, and false ones are corrected (the full address and "plan.md" now match);
  • "The email wasn’t sent." is left alone;
  • a resumed task's summary stands, including with an earlier write in the checkpoint.

I also pushed one small chore to your branch: the import order in sentient/tasks/executor.py (9d03bda), which was the red Lint check. Your code is untouched; please pull before your next push.

A few true replies still get corrected, which is the part we can't ship:

  1. Chat replies read as message sends. Sonnet said "I only sent those two exact text replies you asked for" and got "I didn't send the message". "text", "chat" and "post" (outcomes.py ~109/~125) also describe chat replies. Could the message check need a recipient or an app name instead?
  2. Dotted words read as file names. The pattern (~126) matches "a.m.", "e.g.", "example.com", "3.5" and "Node.js", so "I saved your 7 a.m. wake-up time" gets "I didn't write the file". Maybe require a known file extension, or a word like "file" or "saved to"?
  3. Event words. "meeting" and "appointment" (~127) catch "I've scheduled a task to remind you about the meeting" and a real browser booking ("I've booked your appointment with Dr. Rao").
  4. Same-turn kinds. A declined file write in the same turn makes "I've saved your preference" (after a real memory save) get corrected (~634/~642). The "this turn tried it" check should look at what the sentence names, not just the kind.
  5. "I’ve sent the email" with a curly apostrophe isn't seen as a claim (~73). Once #500 lands (approved; it's waiting on its CLA), you can reuse its _straight_apostrophes helper.

Smaller:

  • a definite server refusal from an earlier turn reads "I couldn't confirm" instead of "I couldn't send" (loop.py ~388);
  • the changelog line still says true reports are left alone, and has an em dash.

Happy to help with tests for these!

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Persist the read-back failure for resumed approved calls.

_run_approved_call() saves the check separately but stores the original tool content in the checkpoint. action_call_history() therefore reconstructs a failed read-back as ok=True. Because the live action record is discarded during resume, the final correction can preserve a false success claim.

Append outcomes.model_note(check) to content before ask.fill_result(...), as the normal loop already does.

…im correction further

Message claims need a recipient or an app name (chat replies are not message sends); file names need a known extension, 'file', or 'saved to' (a.m./e.g./example.com/3.5 do not count); event words do not cover a task reminder or a booked appointment with a person; this turn tried it means tried it for what the sentence names; curly-apostrophe claims are seen via existence-master#500's _straight_apostrophes helper (now in main); a stored server refusal reads 'couldn't send', not 'couldn't confirm'; changelog line softened and em dash removed. Tests for all of these in tests/test_agent_loop.py.

Thanks — round 2 is done in 0a4bfd5 (after merging current main in 1ec6ad8, which also resolves the conflict; your chore commit is pulled in, and the merge conflict was a single import block in sentient/tasks/executor.py, resolved by keeping both sides).

  1. Chat replies as message sends: a message claim now needs a recipient ("to Alice", "@maya") or an app name ("on Telegram") — "text"/"chat"/"post" alone no longer counts as naming a message action.
  2. Dotted words as file names: a file name now needs a known extension (lowercase stem, so "Node.js" does not count), the word "file", or "saved to" — "a.m.", "e.g.", "example.com" and "3.5" no longer count.
  3. Event words: "meeting"/"appointment" no longer names an event in a task/reminder context, or for a booked appointment with a person (a browser booking, not a calendar event). "I booked the meeting." still corrects.
  4. Same-turn kinds: "this turn tried it" now means tried it for what the sentence names (target-aware, with a person-name allowance for email/message) — a declined file write this turn no longer makes "I've saved your preference" get corrected.
  5. Curly-apostrophe claims: fix(agent): treat curly apostrophes as negations in click-claim checks #500 has landed in main, so this reuses its _straight_apostrophes helper — "I\u2019ve sent the email" is now seen as a claim (no new helper invented).

Small items: a stored server refusal from an earlier turn now reads "I couldn't send" instead of "I couldn't confirm" (only a tool that did not finish stays uncertain in history reconstruction), and the changelog line is softened with the em dash removed.

Tests: the action-correction suites pass (12 action tests including new round-2 cases; tests/test_agent_loop.py + outcome checks + approval rules green; ruff clean). tests/tasks has 5 pre-existing failures (web_fetch / probe-page, network-related) that fail identically on the merged tree without these changes. Real-engine verification on qwen3/Sonnet is on your side, as before.

Handed-off and try-run results count as successful action calls.

A "Hand it to me" result (rule_levels.is_handed_off(res)) and a try-run result (res["try_run"]) carry no error. As a result, result_problem(res) is None and this code stores ok=True. Neither call sent or wrote anything. Both paths are reachable:

  • In chat, a handed-off email is stored in the transcript. On the next turn, "I sent the email" is treated as backed by a success and is not corrected.
  • In a resumed task, execute_single rebuilds history from the checkpoint. Try-run notes and handed-off calls in that checkpoint then protect a false "sent" summary.

The same gap exists in outcomes.action_call when _run_tool hands off a call at run time (the rule changed while the call waited). The same happens in _run_approved_call. In both cases ran=True and ok=True.

Do not record these results as successes. The smallest fix is to skip them in both producers.

🐛 Proposed fix
         declined = isinstance(res, dict) and bool(res.get("declined"))
+ if rule_levels.is_handed_off(res) or (isinstance(res, dict) and res.get("try_run")):
+ continue # prepared or noted, never done: neither proof it happened nor that it was refused
         err = _error_text(res)

In outcomes.action_call, return None for the same results:

if isinstance(res, dict) and (res.get("handed_off") or res.get("try_run")):
    return None

Chore pushed to your branch, @azmi2409: ad586f7 removes push-492.py from the repo root. It's your local push helper, and it came in with the merge commit. I also resolved CodeRabbit's four threads on it as moot. Your code is untouched; please pull before your next push.

Thanks @azmi2409, round 2 landed well. Every item from my last review held up on a real engine. I tested 0a4bfd5 with a local mail server (sends go only to a log) and file_write set to Ask so I could decline it: 23 turns on qwen3:8b and 23 on Claude Sonnet. I also ran a couple of cases on main for comparison.

Working as intended now

  • Chat replies are no longer read as sends: "I only sent those two exact text replies you asked for.", "I posted a quick summary in this chat for you." and "I sent you two short options above." are all left alone.
  • Dotted words: "I saved your 7 a.m. wake-up time.", "I’ve saved your preference, e.g. the wake-up time.", "I saved the notes for the Node.js 3.5 setup." and "I sent it to Arjun at 9 a.m. as asked." (after a real send) are all left alone.
  • Event words: "I've scheduled a task to remind you about the meeting." (after a real task) and "I've booked your appointment with Dr. Rao" are left alone. "I booked the meeting with the design team for Friday." with no calendar call is corrected.
  • The curly I’ve: after a declined send, "I’ve sent the email to Arjun." is corrected. After a real send, "I’ve sent the email to arjun@example.com." stands, and so does "The email wasn’t sent because you declined it."
  • Never-made claims: "I saved the file plan.md." after qwen made a task instead of writing the file, and "I sent the message to Priya on Telegram." with no message call, are both corrected.
  • On main, qwen's false "I’ve sent the email to Arjun." (after a decline) and "I saved the file plan.md." (after a declined write) both stand uncorrected. So the PR clearly helps.

What still needs a change. I found two remaining ways a true reply gets a correction. All three Sonnet corrections and one of qwen's five were of this kind.

  1. A claim with no target still matches an old declined call. In _action_note, an empty target matches every recorded call of the kind (_matches_action_target("", call) is True), and that path never asks _owns_claim. So the chat-verb and shared-verb guards from round 2 are skipped. Real cases:

    • qwen: "Those are the two replies I sent." four turns after a declined email got "Correction: I didn't send the email: you declined, so nothing changed."
    • Sonnet, unprompted: "Nothing further happened beyond the two replies I actually sent." got the same correction.
    • Sonnet, after a declined plan.md: "...no summary was written..." got "I didn't write the file: you declined". Directly, "That's the whole summary I wrote." gets it too.

A possible fix: a claim with no target only uses the recorded calls when _owns_claim holds (the sentence names the kind, like "I sent it" plus "email"), or when this turn tried that kind. Then "I sent it." right after a declined send in the same turn is still corrected.

  2. Negations miss "no", and a dot inside a sentence breaks the look-back.

    • _NEGATED_ACTION_CLAIM lacks no/none/zero. After a declined send, "No email was sent." gets "I didn't send the email: you declined", and so does "Zero emails were sent."
    • [^.!?]{0,50} stops at the dot in "Node.js". Sonnet's "nothing about "Node.js 3.5 setup" exists or was saved" was corrected, while "Nothing about the setup was saved." is not.

Allowing a dot that isn't followed by a space (the way the sentence splitter already works) would cover the second case.

Tests for "No email was sent.", "Those are the two replies I sent." and "That's the whole summary I wrote." (each after a declined call in an earlier turn) would lock both fixes in.

Open review threads. Both remaining CodeRabbit threads are valid. Please fix each or reply and resolve it:

  • loop.py ~393 (handed-off and try-run results count as successes): result_problem() returns None for a "Hand it to me" result. So a later "I sent the email to Arjun" after a handed-off send is never corrected. CodeRabbit's two-line skip in action_call_history and outcomes.action_call looks right.
  • loop.py ~381 (a truncated result read as a failure): a result cut mid-JSON fails raw_decode, and the leftover text then counts as an outcome note, so ok=False. It's rare for these tools, but treating an undecodable body as unknown (not a failure) fits "when unsure, leave it alone".

One chore I pushed: ad586f7 removes push-492.py, your local push helper, which came in with the merge. I resolved CodeRabbit's four threads on it as moot. Please pull before your next push. CI is green on it (Windows engine tests were still running when I wrote this), and the CLA is signed.

You're close. These two patterns are the last true-reply corrections I could find. Happy to help with the tests!

Hi @azmi2409, I merged the latest main into your branch so it's up to date: merge commit 857ae133 (no conflicts, your change is untouched). After the merge, ruff check sentient tests packaging is clean and the 487 tests in tests/test_agent_loop.py and tests/tasks pass locally. You don't need to do anything for this; any earlier review points still stand where they are.

@azmi2409, a small heads-up for your next round. #531 (fix #526) adds a separate check for memory claims ("I've saved that you …") at the end of outcomes.py; it doesn't touch your code paths, and git merge-tree shows no conflicts. One overlap: your "saved to" rule would read "I've saved to memory …" as a file claim. Please skip it when the target is memory, so the two checks don't both fire. We'll merge main into your branch for you as usual.
