chore: run the installer from GitHub instead of npm (1.8.8) - #99

The npm account for fullops-squad cannot be recovered, so the installer is no longer published there. `npx --yes github:tourbut/Fullops-Squad-plugin` runs bin/fullops-squad.cjs straight from main, so a merge is the release. package.json is private to stop accidental publishes; README, development docs and update-fullops point at the new command.
