ci(hooks): upgrade discipline to v0.14.4 and enforce agy hook - #64

Bump the CI sentinel, Copilot cloud-agent setup step, and Claude Code cloud bootstrap to discipline v0.14.4 (commit 410daf24), regenerated with `discipline hook install --upgrade`. Remove --observe from the agy Stop hook so all three agents (Claude Code, Copilot, agy) enforce.
