ci: adopt orieg/discipline v0.14.2 with agent hooks in observe mode - #61

Pin the sentinel action to the v0.14.1 release commit, judge changes by the base branch's discipline.toml (policy_from: base), and read override directives from the PR body (and the merged PR body on push). Add the Claude Code PostToolUse/Stop discipline hooks in observe mode plus the cloud-session bootstrap script; un-ignore those two paths in .gitignore.
Pin the sentinel action to the v0.14.2 release commit and regenerate the Claude Code bootstrap hook (pins v0.14.2; puts ~/.local/bin on PATH via CLAUDE_ENV_FILE in cloud sessions).
