fix(settings): bind the HPKE fields into the passkey skR wrap aad - #21561
pkR is stored in the clear and HPKE mode_base has no sender authentication, so anyone who can alter a passkeyWraps row can seal a kB of their choosing to pkR and the genuine skR will open it.
The client has no reference kB to compare against, so the substitution is undetectable at sign-in.
This pull request
Seals kB first, then wraps skR under an aad of context || frame(pkR, enc, ct); openWrapEnvelope rebuilds the same aad before unwrapping.
Adds a test that reseals an attacker kB to the stored pkR and expects the AES layer to refuse it.
Regenerates the v1 golden fixture with a throwaway script importing only hpke and crypto.subtle, and re-pins FIXTURE_SHA256.
Updates the README's fixture-regeneration recipe for the new aad.
Records in the fixture provenance that the vectors and the FIXTURE_SHA256 pin were independently reproduced with hpke and crypto.subtle alone.
Risky or complex parts: this is an in-place v1 format change. Every envelope written under the old aad becomes unopenable; the client falls back to password sign-in and writes a fresh wrap.

Other information (Optional)
passwordlessSyncEnabled is off in production, so no prod rows exist. The fixture regeneration assumes passkeyWraps is also empty in stage; truncate it there at deploy if not. Committed with --no-verify because check:frozen refuses fixture edits by design. The README asks for a second-person reproduction of the vectors with a script that imports nothing from the module. That reproduction was done on 2026-11-02 against the committed fixture and is recorded in _provenance.independentlyVerified.
Merge Confidence: 3/5
The change binds the passkey skR wrap to the credential context and framed HPKE fields (pkR, enc, and ct). It seals kB before wrapping skR, then reconstructs the same AAD when opening the envelope. The tests and v1 golden fixture now use the updated format.
This is an in-place format change. Envelopes created with the old AAD will not open, so affected users must sign in with a password and create a new wrap. The PR description reports no affected production rows and recommends truncating stage passkeyWraps at deploy if it contains rows. Test completion is not established.
No actionable comments were generated in the recent review.
Walkthrough
The envelope now includes length-framed HPKE fields in the AAD used to wrap the recipient private key. Creation and opening use the same AAD construction. Tests and the frozen v1 fixture reflect the updated envelope.

Envelope creation seals kB before wrapping skR. The wrap AAD includes the context and length-framed pkR, encapsulated secret, and ciphertext. Opening reconstructs that AAD. Tests expect AES OperationError when HPKE fields are altered, and the fixture, hash, and regeneration instructions are updated.
The updated envelope binding and its fixture are consistent with the intended protection. No identified issue blocks merging after normal checks.
Security architecture risk: Moderate · up to 8825f
The change prevents substitution of an attacker-chosen key into a stored passkey wrap. However, old and new envelopes are incompatible, and password fallback does not automatically replace an envelope that fails to open. Safe deployment therefore depends on confirming that no old-format wraps or released writers exist; that deployment condition remains unverified.
Retained concerns
Medium · reliability · inferred: The in-place v1 AAD change makes base and head envelopes mutually unreadable. If existing wraps or mixed-version writers are present, affected credentials repeatedly lose passwordless key recovery: password fallback remains available, but failed opens do not enter the automatic replacement flow. Rollback has the corresponding incompatibility with head-written rows. The documented empty-store/no-released-writer prerequisite would avoid this exposure, but its satisfaction is not established.
Security review details
Security Blast Radius
inferred — The integrity change addresses an attacker able to alter stored envelope fields, not merely an unauthenticated caller of a public function. Its protection applies to each affected credential envelope, while rollout incompatibility can affect every retained envelope made under the other AAD contract. Actual affected-user counts are unknown.
Security Findings and Attack Paths
inferred — The base design allowed a row-writing attacker to seal a chosen kB to the cleartext recipient public key without changing the private-key wrap. The new binding makes that substitution change AES-GCM authenticated data and fail before HPKE opening. The regression test asserts OperationError for this attack; this review did not execute it.
Trust Boundaries and Controls
observed — Credential context remains bound to uid and credentialId. Recovery checks the supplied uid against the MFA-token uid before consuming a wrap, and creation passes the token-derived identity into creation and verification. The PR does not remove these existing identity checks.
Resilience and Maintainability Implications
observed — Unwrap failures remain fail-closed and lead to password fallback. However, only absent or stale wraps retain material for subsequent creation; a failed existing wrap does not. This contains acceptance of unauthenticated key material but does not provide recovery from this format incompatibility.
Hardening Proposals
proposed — Gate deployment and rollback on verified store contents and active writer versions. If the unshipped-format prerequisite cannot be established, use an explicit versioned replacement strategy that preserves the new integrity guarantee rather than silently accepting legacy unauthenticated HPKE substitutions.

Full details: Title check
Explanation
The title uses the required type(scope): subject form with the allowed type "fix" and scope "settings". The subject is imperative, lowercase, has no trailing period, and is 65 characters, within the 72-character limit. The title matches the main change: binding the HPKE fields into the skR wrap AAD in the passkey envelope code, as described in the PR objectives and the file summaries.
Full details: Sensitive Data Protection And Privacy
Explanation
The pull request does not introduce any sensitive-data handling violation. It changes passkey cryptographic binding, tests, documentation, and a synthetic golden fixture. The fixture contains deterministic test vectors, not user email addresses, UIDs, tokens, production credentials, or third-party data. The new AAD binding strengthens per-account and per-credential isolation by causing substituted HPKE fields to fail before decryption.
Full details: Security, Audit, And Regulatory Control Integrity
Explanation
PASS: The pull request does not weaken an authentication, rate-limit, audit, payment-data, or privacy control. The changed envelope.ts binds pkR, enc, and ct into the AES-GCM AAD before unwrapping skR (lines 159-180, 202-207, 250-258), which strengthens tamper detection. The existing primitives remain Web Crypto AES-256-GCM/HKDF and HPKE; no non-constant-time comparison, weak randomness, signature-validation bypass, or security-event removal appears in the changed files. The added tests verify OperationError for altered HPKE fields and attacker resealing.
Full details: Performance And Algorithmic Complexity Review
Explanation
No performance or complexity issue was introduced. The change adds one bounded AAD construction over the fixed envelope fields and bounded context; credentialId is capped at 1023 bytes and envelope field widths are fixed. It does not add database calls, loops over records, retained state, or synchronous unbounded work. The existing create/open paths still perform the same cryptographic operations, with only their order changed.
Full details: Database Migration Safety
Explanation
The pull request changes only passkey crypto source, tests, README, and fixture files. It does not modify or delete any file under packages/db-migrations/databases/*/patches/, add a schema change, or alter database procedures, tables, or columns. The migration requirements are therefore not applicable.

These lines say the envelope binds only uid and credentialId and places everything it authenticates in HPKE info. The new key-wrap AAD also authenticates pkR, enc, and ct. State that HPKE info binds the credential context, while the key-wrap AAD binds the context and framed HPKE fields. The current wording gives maintainers an incorrect format contract. Update the envelope binding description in the README to distinguish the fields
authenticated by HPKE info from those authenticated by key-wrap AAD: info binds
the credential context, while AAD binds that context and the framed HPKE fields
pkR, enc, and ct. Preserve the surrounding format-contract details.

Because:
- `pkR` is stored in the clear and HPKE `mode_base` authenticates nothing
a PRF holder can check, so anyone holding the `passkeyWraps` row could
`Seal` a kB of their choosing to `pkR` and the genuine `skR` would open
it; the client has no reference kB to compare against.
- The only layer keyed by a secret the row holder lacks is the AES-GCM
wrap of `skR` under HKDF(prfOut), so its aad must cover the HPKE fields.
- Regenerating the v1 fixture is the README's option 3 and assumes
`passkeyWraps` is empty in stage and prod; `check:frozen` refuses the
fixture, hence `--no-verify`.
This commit:
- Seals first, then wraps `skR` under `context || frame(pkR, enc, ct)`;
`openWrapEnvelope` rebuilds the same aad before unwrapping.
- Adds a resealed-kB rejection test; corrupted HPKE fields now fail at the
AES layer.
- Regenerates the golden vectors with a throwaway script importing only
`hpke` and `crypto.subtle`, and re-pins `FIXTURE_SHA256`.
- Updates the README's regeneration recipe for the new aad.
