B3a: global method cache in front of the chain walks #400

Part of #384. Global (class, name) -> method cache in front of lookup_method_uncached / lookup_class_singleton_method, validated by method_gen (CRuby: vm_method.c global method cache). 7.5% of Rails hello-world samples are in these walks.
