Introduce DocumentMatcher, a reusable base class that extends PathPatternMatcher with document-level matching criteria:
Matching criteria are combined with AND; within each criterion, OR. GlobMatcher utility moved to org.eclipse.lemminx.settings package.
6 subclasses migrated from PathPatternMatcher to DocumentMatcher:
XMLFormattingProfile extends DocumentMatcher with 24 boxed format override fields (flat structure, no nesting). Stored in XMLFormattingOptions.profiles, resolved in XMLFormatter via first-match-wins.
