Sync Cursor pstack 0.15.1 reply evidence rule

Cursor pstack 0.15.1 landed in cursor/plugins PR #341 at f8abeddd1862dc73704e3d719dd73df0d51b8c71. This is the single pstack content change after the 0.15.0 sync completed by #61 and PR #60.

Gavel review on PR #64 found that keeping the already-published Open Pstack version at 1.4.0 would prevent existing installations from receiving the new rule through the normal marketplace update path. The user accepted that finding and expanded this issue to release the sync immediately as Open Pstack 1.4.1.

• Publish the sync as Open Pstack 1.4.1, updating UPSTREAM.md and all three version-bearing manifests together.
• Update NOTICE.md, CHANGES.md, README.md, and docs/reference.md so the package version and Cursor sync point agree.
• Do not change playbooks, model-matrix files, runtime behavior, or dependencies.
• Pass Bun tests, strict typecheck, static invariants, manifest parsing, Claude plugin validation, and git diff checks.
• Install the exact candidate and confirm the new rule from real Claude Code and Codex surfaces before merge.
• Merge the reviewed exact head, tag and publish v1.4.1 from the merged main commit, and prove an existing 1.4.0 installation can take the normal marketplace update and load the new rule.
