TAN-989: Investigate and Fix LeetCode question error in prod

Description of changes

findings:

• LeetCode problem 596 current title updated from "classes more than 5 students" to "classes with at least 5 students" so this outdated value in one of the production submissions was what was causing the lookup to fail (there was 7 submission with the new slug & problem name)

changes:

• Add an isNotFound() flag to LeetcodeClientException for HTTP 200 responses containing data.question: null without GraphQL errors.
• skip topic processing for those questions during the current scheduler run without logging or counting them as errors
• preserve historical submission metadata (database changes)

Replace the problem-596 metadata correction with a general approach that preserves historical submission data. The migration adds an exclusion flag --> it does not update any submission’s title, slug, link, description, or code.
