feat: record the storage account per blob version and resolve blob locations from named accounts - #1187
Storage derives every service owner's storage account name from a configured pattern. Service owners
are moving to accounts whose names do not follow that pattern, and blobs written before and after a
move live in different accounts, so the location has to be recorded per blob version.
This adds storage.storageaccounts (SMALLINT identity id, provider column, provider-specific providerinfo jsonb document, org, storageaccountnumber, writable) and a nullable storageaccountid on storage.dataelementblobversions (foreign key added NOT VALID so the migration
does not scan the table under an exclusive lock). IBlobRepository is keyed by a StorageAccount
instead of an org and account number.
Writes resolve the writable account row for (org, storageaccountnumber) and record its id on the
version row; with no row the naming pattern applies as before ("pattern default"), so behaviour is
unchanged in every environment until operations insert a row.
Reads and cleanup resolve the account from the version row: readdataelement_v3, readinstance_v3
and applyinstancemutation_v2 return the current version's account id, org and number with each
element. Elements from the paged instance query do not carry them, and the resolver refuses such an
element rather than guessing.
Legacy non-versioned blobs stay on the pattern default. The A1/A2 migration writes, deletes and
on-demand reads route through the resolver like every other path, keyed by their blob-storage org
(ttd when A2UseTtdAsServiceOwner is on).
StorageAccountProviderInfo is a discriminated union chosen from the provider column
(azure-blob today, file-system variant prepared for the next PR); the document holds only that
provider's properties. All rows are read once at startup so a row this build cannot read fails the
deployment instead of a download.
The file-scan queue message gains storageAccountProvider, storageAccountProviderInfo and storageAccountId (additive; the scanner ignores unknown properties). Altinn/altinn-file-scan must
prefer the explicit location before any account row is inserted where scanning is on.
Azurite requests are now signed with AzureStorageConfiguration:AccountName; the previous code signed
with the unformatted pattern string and only worked when it happened to equal devstoreaccount1.
A missing Application.StorageAccountNumber means number 1, applied once where the application is
read; new version rows record 1 instead of NULL.
Also in this branch: the unit-test process leaked every disposed test host (the bootstrap console
logger in Program.cs was never disposed and HttpClient handler expiry timers held each host's
service provider), growing past 2 GB and getting OOM-killed on small machines. The bootstrap logger
factory is now disposed when the entry point returns and test hosts use an infinite handler lifetime,
with a regression test that a disposed host is collected. Can be split into its own PR.
Not in this PR: write routing is in (a row for a pair routes new writes at once), but the admin API,
the file-system provider, re-pointing existing versions and backfilling old rows are separate PRs.
Build, CSharpier and the full test suite pass locally (1559 tests, three consecutive full runs after the test-host leak fix)
origin/main's test suite passes against a database already migrated to v0.39 (rollback safety)
Migration timed on 3 million version rows: ~50 ms, FK NOT VALID, lock_timeout 10 s
DB + Azurite integration tests: routing to a second container, old versions still served from the
pattern container, cleanup across two containers, mutation API staging
System tests (test/UnitTest/SystemTests/StorageAccountSystemTests.cs, real Postgres + Azurite): existing
instances get new versions in a second container after the account row is inserted, old versions still
served from the pattern container, cleanup per container, failure paths leave no dangling rows, two hosts
converge within the cache TTL, parallel uploads during the row insert, blob-version audit
(test/UnitTest/Utils/BlobVersionAudit.cs)
File-scan queue message captured from Azurite and parsed with the scanner's own DataElementScanRequest
Documentation
altinn-studio-docs still says "one storage account per application owner"; updated with the operations PR
## System tests requested by the user (run when the portions are in)
1. After portion 1: existing instances get new versions after a writable account row for a second
container is inserted; old versions still read from the pattern container; cleanup per container.
2. After portion 3: some orgs on the file-system provider in parallel with others on Azurite.
3. Additional checks (see the plan's verification section): old code on the new schema, migration
timing on a large table, consistency audit script, concurrency, failure paths, two instances on one
database, cleanup across accounts, file-scan message deserialized with the scanner's own class,
contract snapshots, EXPLAIN ANALYZE of readinstance_v3. Cypress is not set up for local storage: skipped.

## Summary by CodeRabbit
* **New Features**
* Blob versions can now use named storage accounts. New uploads use the selected writable account, while reads and cleanup follow the account recorded for each version.
* Added configurable storage-account cache duration; changes to writable accounts become visible across service instances as the cache refreshes.
* **Documentation**
* Added guidance on storage-account selection, account changes, cache behavior, and moving an owner’s blobs between accounts.

Adds storage.storageaccounts with a jsonb provider document and a
nullable storageaccountid on dataelementblobversions. New function
versions return the current version's account id, org and number with
each data element; a resolver maps them to a StorageAccount, falling
back to the naming pattern when no account row is referenced.
IBlobRepository takes the StorageAccount a blob lives in instead of an
org and account number. Writes resolve the writable account for the
org and number and record its id on the version row; reads and cleanup
resolve the account from the version row; legacy and A1/A2 paths use
the naming pattern. The file-scan request carries the account written
to. Azurite requests are signed with the configured account name.
The provider is a plain column next to the jsonb document, which holds
only that provider's properties; jsonb reorders keys, so a discriminator
inside the document is not relied on. All rows are read once at startup
so a row the running build cannot read fails the deployment. The
file-scan request carries the provider name and document separately.
Adds the version-row foreign key NOT VALID behind a lock_timeout so the
migration cannot stall writers; validates StorageAccounts:CacheSeconds at
startup; compares the Azurite endpoint as a full URI before signing with
the shared key; routes the A1/A2 migration writes, deletes and on-demand
reads through the resolver like every other path; and covers cleanup
across two accounts and mutation-API staging with integration tests.
The unit-test process grew to over 2 GB because disposed test hosts stayed
reachable: the bootstrap console logger in Program.cs was never disposed and
its processing thread held the host builder, and HttpClient handler expiry
timers held each host's service provider for the two-minute handler lifetime.
On an 8 GiB machine the kernel OOM killer ended the test host mid-run.
Dispose the bootstrap logger factory when the entry point returns, give test
hosts an infinite handler lifetime, and add a test that a disposed host is
collected.
Run the service against Postgres and Azurite: existing instances get new
versions in a second container after a writable account row is inserted,
old versions still download from the pattern container, cleanup deletes
each blob from its own container, failure paths leave no dangling version
row, two hosts converge within the cache TTL, and a blob-version audit
checks every version row against the configured containers.
The change adds database-backed storage-account records and associates account IDs with blob versions. A resolver selects named accounts or pattern defaults. Blob reads, writes, scanning, and cleanup now receive resolved storage-account values. Tests and documentation cover account routing and operations.

Pods running the previous release call the unversioned readdetachedblobversions,
readorphanblobversionsforcleanup and readharddeleteddataelementsforcleanup.
They resolve the account from org and number only, so a version stored in a
named account would be looked up in the pattern account, the 404 treated as
deleted and the version row removed, leaking the blob.
The v1 bodies now leave out versions with a storageaccountid. The hard-deleted
reader leaves out the whole element when its attached version has an id, since
returning it without versions would let old code delete the element without
its blob. The _v2 bodies are unchanged.
One storage account row this build cannot read (unknown provider, or a
document invalid for its provider) used to make ReadAll throw. The startup
check then stopped the pod from starting, and on running pods the resolver
refresh failed, so nothing was cached and every blob read and write failed
until the row was fixed.
ReadAll now returns readable and unreadable rows separately; an unreadable
row keeps its id, provider, org, number, writable flag and the error. The
startup check and every resolver refresh log a critical error per
unreadable row and carry on. GetById of an unreadable row, and ForWrite for
the pair an unreadable row is writable for, throw InvalidOperationException
naming the id, provider and error instead of falling back to the pattern
default. Other ids and pairs are unaffected. A failure to read the table
itself still propagates.
…blob versions
New blob version rows record the application's StorageAccountNumber as it
is, NULL when the application has none, as the previous release did. The
write paths still resolve the account with the defaulted number. Old and
new pods therefore agree on rows without a number regardless of how
OrgStorageAccount is spelled.
GeneralSettings:StorageAccountCacheSeconds (default 60) replaces
StorageAccounts:CacheSeconds, and the separate StorageAccountSettings
class and its validator are removed. The resolver rejects a value that is
not greater than zero.
The azure-blob provider info document held an optional endpoint, and the
blob repository sent the managed identity token to any endpoint other
than the configured Azurite one. The document now holds only accountName
and container, both validated against the Azure naming rules, and an
unknown property (such as endpoint) fails deserialization.
AzureBlobLocation derives the location in one place: devstoreaccount1 is
the configured BlobEndPoint with the configured shared key (and is
rejected unless Azurite is configured); every other account is
https://{account}.blob.core.windows.net with DefaultAzureCredential.
The pattern default lowercases the names it formats from the org.
Describes the table, the per-pod cache, how reads resolve the account
of a blob version, how uploads choose the writable account, cleanup per
account and how an owner's blobs are moved by copy and re-point. Linked

🟡 Changes recommended
Write-account resolution runs even for metadata-only migrations, causing requests that never write blobs to fail when the writable account is unreadable.
Adds per-blob-version storage locations so new uploads can use explicitly named accounts while existing blobs remain readable from their original accounts.
Changes:
Adds storage-account persistence, version references, and account-aware PostgreSQL readers.
Routes uploads, downloads, signing, migration, and cleanup through a cached account resolver; adds explicit locations to file-scan messages.
Corrects Azurite signing and default account-number handling.
Updates tests, mocks, operational documentation, and test-host resource disposal.
This pull request requires changes. Unresolved moderate findings affect instance and element cleanup after pattern-account access is removed, metadata-only migration requests, and concurrent cache refreshes. Documentation nits concern unsupported file-system activation, account-move sequencing, continued pattern-account access, and the startup-validation policy.

Hold one upload open across the row insertion.
Task.WhenAny(uploads) returns after the first phase-one upload completes. InsertWritableAccount then runs. The other phase-one uploads can already have finished, or can still be before account resolution. The test therefore does not guarantee that an upload is in flight when the account row is inserted. The assertions pass for either schedule, so the test can pass without exercising the overlap its name describes.
Add a test-controlled gate after ForWrite for one phase-one upload. Release the gate after InsertWritableAccount completes. This makes the insertion overlap an in-flight write, so the existing assertions check that the blob keeps the account it resolved.

Add system-test coverage for an application with a non-default storage account number.
The system-test application does not set StorageAccountNumber. InsertWritableAccount always registers a writable row for StorageAccountNaming.DefaultStorageAccountNumber. An upload that hard-codes account 1 would still write to the only writable row and pass these tests.
Add a test that:
seeds an application with StorageAccountNumber = 3,
registers a writable row for (org, 3), and
asserts that the uploaded blob is written to that container.
Give the insert helper a number parameter, or add a separate helper for this case. Keep the number-3 setup in the new test. The shared StartHost seed must keep its default number, because InsertWritableAccount registers only number 1 for the other tests.

Require a wait for old-account writes before the final copy and re-point.
Pods can keep writing to the old account for up to one resolver cache period after the new writable row is inserted. An upload that starts in that window can create a version row with the old storageaccountid, then write its blob after the final copy. The re-point step matches that row and switches it to the new account. Reads then go to the new account and fail until the blob is copied there. Add a step that waits for StorageAccountCacheSeconds and drains in-flight uploads before the last incremental copy, and make the re-point run only after that copy.

Continue past one unreadable storage account in cleanup.
A hard-deleted data element with a version in an unreadable storage account makes ForVersion throw. The per-element catch in CleanupDataelements returns a 500 at once. Elements sorted after the failing one, and the orphan blob-version phase, do not run. The failing element is not deleted, so each run stops at the same point until the account row is repaired. Record the failure for that element, continue the loop, and return 500 after the loop. Apply the same per-item handling in CleanupOrphanBlobVersionsInternal.

Skip instance blob cleanup when the pattern-default container is absent.
Instance cleanup deletes the pattern-default prefix before any named-account deletion. When that delete fails, the instance is skipped. Its named-account blobs and metadata are retained, and the next run selects it again. Start the named-account deletion and metadata deletion only when the instance has no version that needs the default location, or treat a missing default as not an error for such instances.
The gate predates this PR. The PR makes named-only instances possible, and its documented account move and retirement procedure can leave the default container absent or inaccessible.

The new ForWrite call runs before the hasBlob check. A bodyless on-demand element never uses that account. If the writable row for the (org, number) pair is unreadable, ForWrite throws, and the catch-all returns 500 instead of creating the element. Move the lookup into if (hasBlob).

Move the `_storageAccountResolver.ForWrite` lookup into the `if (hasBlob)`
branch so bodyless on-demand elements do not resolve an unused blob account;
preserve the existing lookup arguments and blob-version creation flow.

Do not list file-system as a supported provider until BlobRepository can use it.
A storageaccounts row with provider file-system loads as readable and can be selected for reads and writes. Every blob operation then throws NotSupportedException in GetLocation. docs/storage-accounts.md says this provider is reached through StorageAccounts:FileSystem:RootDirectory, but no such setting or file-system implementation exists in src/Storage. Either implement the provider or change the docs to mark it unsupported.

Update the storage-account documentation so it does not present the file-system

AzureBlobProviderInfo; state that file-system accounts cannot currently be used

Check that the data element belongs to the signed instance before hashing.
GenerateSha256Hash loads the element by ID only. It then hashes the blob from whatever storage account the element row names. A caller authorized on one instance can therefore sign a digest for an element that belongs to another organization's instance, stored in another named account. The digest is then written into the caller's signature document. The base code had the same missing check, but it read through the caller's organization account, so cross-account elements usually failed. This change resolves the account from the element, so that restriction no longer applies.

Validate in GenerateSha256Hash that the loaded data element belongs to the

Gate named-account writes until every instance can read them.
ForWrite returns a writable named account as soon as the snapshot contains that row. Instances on the previous release derive blob location from organization and account number only. They cannot read blobs written to a named account. Before activating a writable row, confirm that no previous-release instance serves traffic. Alternatively, add a rollout gate. The docs cover only cleanup compatibility.

Update ForWrite’s writable named-account path so it returns a named account only
after an existing rollout or activation gate confirms no previous-release

When the snapshot is missing or expired, GetSnapshot calls Refresh without a lock or a shared in-flight task. Each concurrent miss runs its own storage.readstorageaccounts() query and rebuilds the snapshot. A burst of uploads or named-account reads at cache expiry can therefore issue one query per request. Store a single in-flight Task<Snapshot> (or guard Refresh with a SemaphoreSlim and re-check the cache after acquiring it) so that one query serves all waiting callers.

Update GetSnapshot and Refresh to coalesce concurrent cache misses using a
shared in-flight refresh task or a semaphore; if using a semaphore, re-check the
cache after acquiring it. Ensure concurrent callers reuse the refreshed Snapshot
so only one storage.readstorageaccounts() query runs per refresh.

Storage derives every service owner's storage account name from a configured pattern. Service owners
are moving to accounts whose names do not follow that pattern, and blobs written before and after a
